Client Spotlight: Duke Health Organization Focuses Resources to Tackle COVID-19 Needs in Local Communities

This is the second article in an ongoing series detailing how some of our clients are pivoting operations and/or contributing to their communities in response to the COVID-19 pandemic. Most companies have had their day to day operations impacted by this pandemic, yet many are still chipping in to help flatten the curve in their own manner. We thought it appropriate to showcase some of that work.

Duke Population Health Management Office (PHMO)

The Duke Population Health Management Office (PHMO) centralizes population health delivery capabilities previously housed at Duke University School of Medicine, Duke Private Diagnostic Clinic, and Duke University Health System. Their goal is to facilitate higher-quality care while lowering healthcare costs. They help individuals, employers, payers, and the community improve health outcomes through the DukeWELL™ care management platform and the Duke Connected Care Clinically Integrated Network, which is a clinically integrated network that aims to improve the quality of healthcare while addressing the national challenge of rising healthcare costs.

The COVID-19 pandemic has been a critical time for individuals and groups affiliated with Duke PHMO to aid healthcare professionals and patients in our community.

This pandemic has changed healthcare. As an organization, we’ve had to be flexible so we can meet the immediate needs of our providers and patients. At the same time, we’re planning for what healthcare will look like after COVID-19. This requires us to be innovative as we strive to achieve our vision of connected patients, empowered providers, and healthy communities.

A few examples of how Duke is helping to flatten the curve:

  • The DukeWELL Care Management team works around the clock to connect with patients via telephone and email in order to get patients connected with medical resources and to follow up after appointments. Team members are doing a majority of the work via telehealth, which is reducing the number of people who come into a facility or hospital, slowing the spread of the virus, and freeing up valuable resources for COVID patients.

  • Duke's Provider Relations team is working with the independent community practices that are members of the Duke Connected Care Clinically-Integrated network to provide patient protective equipment (PPE) from Duke Health stockpiles. These independent providers would otherwise be stuck requesting through government channels. This is one of the benefits for providers joining Duke Connected Care.

  • PHMO staff members are participating in the Duke Community Health Watch Study as part of the Pandemic Response Network. This study connects clinical staff to patients who are recovering from or who have health concerns regarding COVID. It’s another tool PHMO can use to monitor the spread of the virus. So far, the results have concluded the discharged patients are effectively receiving follow-up care and remaining isolated to slow the spread of the virus.
